Hospice Care Bridge exists because one of life’s most profound experiences shouldn’t also be one of its most confusing.

Every day, families are handed a hospice diagnosis with little more than a pamphlet and a phone number. The clinical team is doing their best. But grief, stress, and the weight of new responsibility make it hard to absorb information — even when it’s been clearly explained.

Hospice Care Bridge was built by a licensed nurse with over ten years of hospice experience. The mission is to translate the clinical into the human. To give families the clarity, confidence, and support they deserve.
